In short

Marionette lines run down from the corners of the mouth towards the chin. They usually deepen because the jowl descends beside them, the area in front of the jowl hollows and the mouth-corner muscle pulls down. Marionette lines treatment therefore often starts with lifting, such as VF Lift – Vertical Facelift, with Restore or filler for the hollow and relaxing injections where the muscle pulls.

Key facts
  • What marionette lines are: a validated grading scale describes marionette lines, or melomental folds, as curved lines running down from the corners of the mouth, formed by facial movement and ageing; nine experts used it to rate 35 people with high agreement [1].
  • The jowl sits right beside them: in 30 hemifacial cadaver dissections, the jowl fat was the lowest of the separate fat compartments of the face [2].
  • Bone loss deepens the groove: the prejowl area and the jaw are among the parts of the facial skeleton that resorb predictably with age [3].
  • Combining filler and muscle relaxation: in a prospective, evaluator-blinded study, filler at the mouth corners and marionette lines followed by relaxing injections along the mouth-corner muscle moved the median mouth-corner angle from minus 4 to minus 1 degree at 3 months [4].
  • Filler risks are mostly minor: an international consensus found most hyaluronic acid filler complications mild and short-lived and serious events rare, and most avoidable with planning, anatomy and product choice [5].

The difference

What are marionette lines, and how are they different from smile lines?

Marionette lines run down from the corners of the mouth towards the chin [1]. Smile lines, the nasolabial folds, run from the side of the nose to the mouth corner.

Marionette lines compared with the lines near them. Many faces have more than one.
LineWhere it runsWhat usually drives itWhere treatment starts
Marionette linesFrom the mouth corner down towards the chinJowl descent, the prejowl hollow, a downward-pulling mouth-corner muscleLifting the jowl, then support for the hollow
Smile lines (nasolabial folds)From the side of the nose to the mouth cornerCheek descent and mid-face volume lossLifting or supporting the cheek
Prejowl sulcusA dip beside the chin, below the end of the marionette lineBone and fat thinning in front of the jowlRestore or filler in the dip

The two are often treated in one plan with different steps; smile lines have their own page, nasolabial folds treatment. A marionette line can also give a sad look at rest, often described as a downturned mouth.

Causes

What causes marionette lines?

Mostly what happens next to them: the jowl descends beside the line, the area in front of the jowl hollows, and the muscle at the mouth corner pulls down.

  • Jowl descent. The jowl fat is the lowest fat compartment of the face [2]. As it fills or slides, it sits just outside the marionette line and casts the shadow that makes the line look deep.
  • Bone and fat thinning in front of the jowl. The prejowl area and the jaw resorb with age [3], so the groove below the mouth corner deepens.
  • The mouth-corner muscle. The depressor anguli oris pulls the corner of the mouth down; a strong pull deepens the line and turns the corner downwards.
  • Skin and face shape. Thinner skin creases more easily, and a short chin or significant weight loss makes the lines look deeper.

The full picture of lower-face descent is on jowl treatment in Singapore, and the whole face on sagging face treatment in Singapore.

Lift first

Why does a lifting-first approach often work better?

Because the line is usually a shadow below a descending jowl. Filling the groove without lifting the jowl adds weight to the lower face; lifting first usually means less filler, or none.

When the jowl beside the line is tightened or flattened, the shadow it casts becomes shallower. What remains is then judged on its own: a hollow that may suit Restore or a little filler, and a muscle pull that may suit relaxing injections. This order is the same principle as facial volume restoration: lift first, then fill only what is still missing. It also avoids repeated filler making the area around the mouth look full and heavy.

Decision table

Which marionette lines treatment suits which cause?

Lifting for the jowl, Restore or filler for the hollow, relaxing injections for a pulling muscle and skin treatment for fine creasing. Most faces need two of these.

VF Lift – Vertical Facelift

How does VF Lift – Vertical Facelift help marionette lines?

Lift tightens the skin along the jaw, Sculpt flattens a pouching jowl beside the line, and Restore rebuilds the prejowl hollow with your own fat. Nothing is injected.

Dr Sin Yong maps the lower face zone by zone. Lift is planned along vertical vectors against the direction the face has descended; Sculpt is used where the lower face pouches; Restore, the mode he values most, is customised to hollows including the prejowl sulcus, and helps the patient’s own fat there regenerate, so volume develops over the following weeks. Because nothing is injected, there is no risk of overfilling the area around the mouth. Most plans involve two to four sessions with little downtime. Radiofrequency of this kind and focused ultrasound improved lower-face laxity to a similar degree in a split-face trial [6]. Full details are on VF Lift – Vertical Facelift.

Other lifting

Can Time Freeze Laser LCLR® or a thread lift help?

Yes, for the right face. Time Freeze Laser LCLR® suits fuller faces where fat drags the jowl down; threads suit moderate descent where a direct mechanical lift is wanted.

Time Freeze Laser LCLR® combines dual-wavelength tightening with point-zone fat reduction, with redness for an hour or two. A thread lift in Singapore repositions tissue with absorbable threads; the hold softens as they dissolve, and evidence for lasting lift is limited [7].

Filler

When is filler right for marionette lines?

When a hollow or groove remains after the jowl has been assessed, the skin is in reasonable condition, and only a small amount is needed. It is a finishing step more often than a first step.

Filler can support the mouth corner, soften the groove below it and fill the prejowl dip, and where the chin is short, chin fillers in Singapore can make the whole area look less downturned. Hyaluronic acid filler can be dissolved if needed. How products and depths are chosen is on dermal fillers in Singapore, and the jaw line on jawline filler. Dr Sin Yong performs injectable treatment himself.

Most filler side effects are mild and short-lived, such as swelling, tenderness and small bruises, and serious events are rare [5]. What can go wrong, how risk is lowered and what to do is set out on filler complications; product that has moved or overfilled the area is assessed on filler correction.

After filler

Which signs after filler need urgent attention?

Pale or blanched skin, severe or increasing pain, mottled or dusky skin, or any change in vision after filler need urgent attention: contact the treating clinic immediately.

For any change in vision, go to the nearest emergency department straight away. These signs can mean a blood vessel has been affected, which is rare but time-critical; it is explained in vascular occlusion after filler. Separately, a mouth corner that suddenly droops on one side, with weakness, slurred speech or a drooping eyelid, is not cosmetic: call 995, as this can be a stroke or facial nerve palsy.

Mouth-corner muscle

Do anti-wrinkle injections help a downturned mouth?

They can, where the muscle at the mouth corner pulls it down. Small amounts relax the pull, which can lift the corner slightly and soften the line; they do not fill a groove or lift a jowl.

The muscle, the depressor anguli oris, sits just below and outside the mouth corner. Relaxing it is often combined with a little filler at the corner, an approach studied in a prospective evaluator-blinded study [4]. Placement too close to the lip can affect the smile, so doses are kept small and reviewed at about two weeks; the effect wears off over a few months. Areas and timing are on anti-wrinkle injections.

Skin quality

Can skin treatments help fine marionette lines?

For fine creasing in thin skin, yes. Skin treatments improve texture and firmness; they do not lift a jowl or fill a deep groove.

FSX Laser, the clinic’s non-ablative thulium and erbium glass protocol, uses erbium glass passes that can soften fine lines around the mouth; redness usually settles within hours. Skin boosters improve thin, dry skin, and daily sunscreen protects collagen in Singapore’s sun.

Surgery

When is a surgical opinion the better route?

When the jowl beside the line hangs well below the jawline, the skin falls in folds, or several rounds of filler have already been needed to hold the area.

A surgical lower facelift repositions and removes tissue under anaesthesia. Where non-surgical treatment cannot reach the goal, Dr Sin Yong says so and suggests a plastic surgery opinion; the routes are compared in facelift in Singapore: surgical vs non-surgical.

“A marionette line is usually the shadow under a jowl, not a crease of its own. Fill the shadow without lifting what casts it, and the lower face gets heavier rather than younger.”

Cost

What affects the cost of marionette lines treatment in Singapore?

The cost follows the plan: whether the jowl needs lifting, whether a hollow needs Restore or filler, whether the mouth-corner muscle is treated, and how many sessions. A written plan and cost follow assessment.

  • Lifting: the zones and sessions of VF Lift – Vertical Facelift or Time Freeze Laser LCLR®, or a thread lift.
  • Filler: the amount and product, if filler is advised at all.
  • Anti-wrinkle injections for the mouth-corner muscle, repeated when movement returns.
  • Maintenance, which depends on how your face changes.

How consultation and treatment fees are handled is explained on our fees page. At IN Eternity Clinic a consultation fee applies, and it is waived if you go ahead with any medical treatment; the fee is S$120.

Timeline

How soon will marionette lines look softer, and for how long?

Filler shows on the day once swelling settles; injections over about two weeks; lifting treatments over weeks to months. All need maintenance as the face keeps ageing.

What Dr Sin Yong looks for

  • Whether the line is a crease of its own or the shadow below a descending jowl.
  • The depth of the prejowl sulcus and the projection of the chin.
  • The pull of the mouth-corner muscle at rest and when speaking.
  • Skin thickness and fine creasing around the mouth.
  • Whether the jowl is beyond non-surgical reach, in which case a surgical opinion is suggested.

Questions

Frequently asked questions

Which treatment suits marionette lines?

It depends on the cause. Where a jowl descends beside the line, lifting with VF Lift – Vertical Facelift or Time Freeze Laser LCLR® usually comes first; a hollow may suit Restore or a little filler; a pulling mouth-corner muscle may suit anti-wrinkle injections. Most plans combine two, chosen at assessment.

Can filler fix marionette lines?

Filler can soften a groove and support the mouth corner, especially with good skin and little jowl descent. Where the jowl casts the shadow, filler alone adds weight to the lower face, so lifting is usually considered first and filler is kept small.

Can marionette lines be treated without needles?

Often, yes. VF Lift – Vertical Facelift lifts the jaw line and flattens a pouching jowl, and its Restore mode rebuilds the prejowl hollow with your own fat, with nothing injected. Time Freeze Laser LCLR® and FSX Laser are also needle-free.

What causes marionette lines at a young age?

In younger faces they usually come from face shape rather than ageing: a short chin, a strong mouth-corner muscle, or rapid weight loss. Treatment then targets the cause, such as chin projection or relaxing the muscle, rather than lifting.

How long does marionette lines treatment last?

It varies with the treatment. Anti-wrinkle injections usually last a few months; filler longer, depending on the product; collagen made by lifting treatments remains, though ageing continues. Most plans include periodic maintenance.

Is filler around the mouth safe?

Filler is generally safe, with most side effects mild and short-lived. Serious events are rare but include a blocked blood vessel, so anatomy, product and depth are planned carefully, and the clinic has an emergency protocol. Signs that need urgent attention are listed on this page.

How should I prepare for filler around the mouth?

Tell the clinic about any blood-thinning medicines or supplements, such as fish oil, and any history of cold sores; do not stop a prescribed medicine without your own doctor’s advice. Avoid booking just before an important event, as swelling or bruising can last a few days.

Can marionette lines come back after filler?

Yes. Filler is gradually broken down, and the jowl and bone keep changing, so the line slowly returns. Lifting the jowl first usually means less filler is needed to maintain the result.
