Skin tags are removed at their stalk under local numbing, by fine snip, radiofrequency or CO2 laser depending on size and site. Most take minutes, heal with a small scab in about a week, and do not grow back at the same spot, although new tags can form elsewhere. Each lesion is examined by a doctor first.

Diagnosis

Is it really a skin tag?

Most soft, skin-coloured flaps on the neck are skin tags, but several other growths look similar. Seborrhoeic keratoses can be raised and stalk-like but have a waxy, stuck-on surface (see seborrhoeic keratosis). Soft moles can hang from the skin and sometimes carry a hair. Filiform warts on the face and eyelids look like thin threads but are a viral infection that needs a different plan (see wart removal).

Dr Sin Yong examines each lesion, using a dermatoscope where there is any pigment, bleeding or irregularity. Anything that does not look like a typical tag is assessed separately, and some lesions are sent for laboratory examination after removal. A tag that turns dark purple and painful has usually twisted on its stalk; it is benign, but prompt removal is sensible.

Methods

Snip, radiofrequency or CO2 laser: how each tag is removed

Fine snip. Small tags on a narrow stalk are lifted and cut at the base with sterile fine scissors after numbing. Any pinpoint bleeding is stopped with pressure or a touch of cautery.

Radiofrequency. A fine radiofrequency tip seals and removes tags with a broader base, and is efficient when there are many small tags in one area, such as the neck.

CO2 laser. Ablative CO2 laser removes tissue in very thin layers with fine control of depth. It is useful for flatter tags and delicate sites. IN Eternity Clinic is a designated DEKA Centre of Excellence.

Why they appear

Friction, genes and metabolism

Skin tags become more common with age and run in families. They cluster in skin folds where there is friction and humidity, which is one reason they are so common in Singapore’s climate. Pregnancy and weight gain can bring new ones.

Studies have linked multiple skin tags with insulin resistance and impaired glucose metabolism. A sudden crop, especially with darker velvety skin on the neck or underarms, is worth mentioning, and Dr Sin Yong may suggest a blood sugar check with your GP. Removing the tags does not change that underlying tendency, which is why new tags can appear elsewhere later.

Comfort

Does it hurt? Comfort and numbing

Very small tags are usually treated after numbing cream has been on for 30 to 45 minutes. Larger tags, or tags on thicker skin, are numbed with a small local anaesthetic injection, which stings for a few seconds; after that most people feel only pressure or mild warmth.

Eyelid tags are numbed carefully and treated with the eye shielded. Many tags can be removed in one visit, and the area is numbed in sections so comfort is kept throughout.

Timeline

What to expect: day by day

Day 0. Examination, numbing and removal, usually 15 to 30 minutes in total. An ointment is applied, and a light dressing where clothing rubs.

Days 1–3. Small crusts form over each site. You can shower normally; pat dry and apply the ointment provided. Avoid picking.

Week 1. Crusts lift on their own over about five to ten days, leaving small pink spots.

Weeks 2–4. Pinkness fades. Temporary darkening can occur in Asian skin, so sunscreen is used on exposed areas such as the neck.

Review. Any laboratory result is discussed, and new or missed tags can be treated at a later visit.

Will skin tag removal leave a mark?

Most sites heal as a small pink spot that fades over weeks. Temporary darkening can occur in Asian skin, particularly on the neck, and is reduced with sun protection and by not picking the crusts. Scarring is uncommon when tags are removed at the right depth.

Can skin tags on the eyelids be removed?

Yes. Eyelid tags are removed with fine settings after careful numbing, with the eye shielded throughout. The skin there is thin, so depth is kept shallow and only the tag itself is taken. Healing on the eyelid is usually quick, though mild swelling for a day or two is common, and eye make-up is avoided until the small crusts have lifted.

Can many skin tags be removed in one session?

Usually, yes. Many people have several tags on the neck, underarms and eyelids removed in a single visit. The number treated depends on their size, the areas involved and your comfort with the numbing. The plan and the number of tags are agreed before starting, and any left over can be treated at a short follow-up visit.

Do skin tags need to be sent to the laboratory?

Typical skin tags do not need to be. If a lesion looks atypical, is pigmented, has bled, has grown quickly or has changed in colour, it may be sent for laboratory examination after removal so the diagnosis is confirmed. You will be told beforehand if this is recommended and how the result will be shared with you.

Are home skin tag patches or freezing kits safe?

They can irritate or blister the surrounding skin, often leave the base behind so the tag persists, and skip the step of confirming what the lesion actually is. Some products are made for warts rather than tags. A doctor’s removal is quick and controlled, with numbing, clean technique and aftercare, and a lower risk of infection and scarring.

Cost depends on the number of tags, their size and location, and the method used. Eyelid tags take more time and care than tags on the neck, and many small tags can often be treated together.
