Jowl Fat Treatment in Singapore

Lift it, reduce it, or both

The jowl is the one zone where fat reduction alone makes the face look older. The lift has to come with it.

In short · Doctor’s answer

A jowl is a small pad of fat that has descended below the jawline, together with the loose skin and weakened ligaments that let it descend. Treating the fat alone leaves a loose fold; treating the laxity alone leaves a bump. At IN Eternity Clinic both are treated in the same session with Time Freeze Laser LCLR®: the lipo-modelling pass thins the mapped jowl pad and the lifting pass contracts and remodels the skin and the jawline above it. A muscle relaxant to the depressor muscles at the corner of the mouth is added when they are pulling the jowl down. Advanced jowling with a lot of excess skin is a surgical facelift case.

What the research says
  • Jowling results from descent of the facial fat compartments with ligament laxity and loss of skin elasticity; treatment that addresses both the volume and the support gives the more natural result (facial anatomy literature; Rohrich & Pessa 2007).
  • Laser energy delivered to subcutaneous fat disrupts adipocytes and tightens the skin above (Badin 2002).

Anatomy

What a jowl is made of

The lower face holds fat in compartments supported by ligaments. With age the mandibular ligament holds, the tissue in front of it descends, and a pad forms below the jawline: the jowl. In front of it a small hollow (the pre-jowl sulcus) makes the jowl look larger than it is. The skin over the pad has usually lost elasticity, and the depressor muscles at the corner of the mouth add a downward pull.

So a jowl is three things: a little fat, a lot of laxity, and sometimes muscle pull. The plan has to say which is dominant.

The plan

How jowls are treated at IN Eternity

  • Assess. Pinch test for fat, skin recoil for laxity, grimace for muscle pull; photos.
  • Map. The jowl pad is mapped so that the lipo pass is pointed at fat only.
  • Treat. Lipo-modelling pass on the pad; lifting pass over the jowl, jawline and lower cheek; neck included where the laxity continues downward.
  • Add. Muscle relaxant to the DAO muscles if they pull; a small pre-jowl filler only if a true hollow remains after lifting.
  • Review. At four to eight weeks, as collagen builds; further sessions as a spaced course.

Results vary between people and are reviewed at each visit. Costs depend on the zones and sessions and are set out at the consultation; the consultation fee is S$120 and is waived when a treatment is started.

Honesty

When a facelift is the better answer

Deep jowls with a lot of loose skin, marked neck laxity and heavy platysma bands are a surgical case; non-surgical lifting improves them but will not match a well-done facelift. Dr Sin Yong says so at the assessment, and the LCLR protocol is then used, if at all, to maintain the surgical result and treat skin quality once healing is complete.

Questions

Frequently asked questions

Can jowls be treated without surgery?

Mild to moderate jowls, yes: the fat pad is thinned with laser lipo-modelling and the skin lifted in the same session, with a muscle relaxant for the depressor muscles where they pull. Advanced jowling with much excess skin is a facelift case.

Does HIFU get rid of jowls?

HIFU lifts by heating the SMAS layer and improves mild jowling, but it does not reduce the fat pad itself. Where the jowl is partly fat, a lipo-modelling pass is added.

Will reducing jowl fat make my face look older?

If the fat is reduced without lifting, yes: the loose skin remains as a fold. That is why the fat and lifting passes are done together.

How many sessions?

A spaced course set at assessment and reviewed as the jawline responds; collagen builds for about three months after each session.

Does jowl treatment hurt?

Most people describe warmth and light prickling under cooling; the fat zone can feel tender for a day or two.

How long do results last?

Collagen built after each session is yours; the ageing process continues, so periodic maintenance is usual. Weight stability helps the fat result hold.
