Face Changes After Rapid Weight Loss
It is not the drug. It is the speed of the loss and the fat the face gives up with it.
Ask Dr Sin Yong on WhatsApp →"Ozempic face" is the hollow, tired, older look that can follow rapid weight loss from any cause, including GLP-1 medicines such as semaglutide. The face loses fat from the cheeks, temples and under the eyes faster than the skin can shrink, so hollows and loose skin appear together. It is a result of the weight loss, not of the drug. During treatment, slower loss, enough protein and resistance training limit it. Afterwards, at IN Eternity Clinic, the laxity is treated with Time Freeze Laser LCLR® lifting, thinning skin with collagen stimulators or skin boosters, and true hollows, if they remain, with a small volume of filler placed deep.

- Rapid weight loss reduces facial fat compartments and skin support, producing hollowing and laxity; facial plastic surgeons report increased presentations after GLP-1 use (Humphrey 2023).
- In STEP 1, about a quarter to a third of total weight lost on semaglutide was lean mass, which is why protein and resistance exercise are advised (Wilding 2021).
Why the face ages with fast weight loss
Facial fat sits in compartments that give the cheeks and temples their shape. Lose ten to twenty kilograms over a few months and those compartments shrink while the skin, which has been stretched for years, has no time to contract. The result: hollow temples and cheeks, deeper tear troughs and nasolabial folds, jowls that appear because the cheek no longer holds the tissue up, and a general loss of the "fullness" that reads as youth.
The same happens after bariatric surgery or any strict diet; GLP-1 medicines have made it common because they make large losses easier.
Limiting it during treatment
- Lose at the rate your doctor sets rather than faster; the dose build-up over four months exists partly for this reason.
- Protein at every meal (about 1.2–1.6 g per kg per day) and resistance training twice a week to keep muscle, including the muscles of the face and neck.
- Sun protection and a retinoid at night to keep the skin’s own collagen production up.
- Hydration and sleep, which affect how hollows look day to day.
- Wait until weight has been stable for two to three months before treating the face; treating earlier is treating a moving target.
What restores the face afterwards
| Problem | First-line at IN Eternity | Also |
|---|---|---|
| Loose skin, jowls, soft jawline | LCLR lifting pass (face and neck) | HIFU for deep laxity |
| Thin, crepey skin | Collagen stimulators (Sculptra); skin boosters | Thulium pass of LCLR |
| Residual hollows after lifting | Small volume of HA filler, placed deep | Sculptra for diffuse volume |
| Submental fullness that stayed | LCLR lipo-modelling pass |
The order matters: lift first, then judge the hollows. Most of what looks like volume loss is descent, and filling descent makes the face heavier, not younger. Only a true deficit that remains after lifting is filled, with a reversible product.
Cost and timing
Face treatment is planned once weight is stable, usually two to three months after the maintenance dose is reached or after treatment ends. Costs depend on which problems are present and the sessions needed; they are explained at the consultation. The consultation fee is S$120 and is waived when a treatment is started.

Is Ozempic face caused by the drug?
No. It is caused by rapid loss of facial fat and the skin not shrinking at the same pace. Any fast weight loss does it; GLP-1 medicines make fast loss common.
Can Ozempic face be prevented?
Largely limited: a gradual rate of loss, enough protein, resistance training, sun protection and a retinoid. Some facial fat loss is inevitable with a large weight change.
Will my face recover if I stop the medicine?
Regaining weight refills facial fat, which is not the aim. The better route is to keep the weight and treat the laxity and any true hollows directly.
Should I get filler for Ozempic face?
Usually only after lifting, and only for hollows that remain. Filling descent makes the face heavier. A small deep volume of a reversible filler is enough when needed.
When should I treat my face?
Once weight has been stable for two to three months. Treating during active loss means treating a moving target.
Does the laser also help the body skin?
Loose skin on the body after weight loss is treated with the body zones of the same laser protocol or with microwave contouring; large amounts of excess skin need surgery.
