In short

Sculptra aftercare centres on massage, often five minutes, five times a day, for five days, plus avoiding heavy pressure, heat and strenuous exercise early on. Early fullness fades within a week; collagen builds over months.

Key facts
  • The 5-5-5 rule: the maker’s patient information describes massaging the treated area for 5 minutes, 5 times a day, for 5 days, to spread the product evenly [1].
  • Technique and massage go together: a consensus of experienced injectors recommended higher dilution, fewer vials per session, placement under the skin rather than in it, at least six weeks between sessions, and massage after injection to reduce complications [2].
  • How it works: poly-L-lactic acid microparticles trigger a mild inflammatory response that stimulates collagen deposition, so the result builds gradually rather than on the day [3].
  • What patients may notice: the maker lists pain, redness, bruising, tenderness, itching, lumps, bleeding and swelling at the injection site, and notes that small bumps can sometimes be felt when pressing [1].
  • Late lumps are possible: late-onset and recurrent facial nodules after poly-L-lactic acid injection have been reported, which is why lumps that appear later need medical review [4].
The short answer

What is the 5-5-5 rule after Sculptra?

It is a massage routine: five minutes, five times a day, for five days after each session. It helps the poly-L-lactic acid particles spread evenly while they settle [1].

Sculptra is not a gel filler. It is a suspension of microparticles placed deep, in the fat layer or just above bone, that the body responds to by building collagen over months. What it treats and how a course is planned is on Sculptra in Singapore; at IN Eternity Clinic, Dr Sin Yong shows the massage routine at the end of each session and writes it down. Related guides are under injectables.

Why massage

Why is massage important after Sculptra?

Because the particles can clump if they settle unevenly, and clumps are the main cause of small lumps. Massage in the first days spreads them across the treated plane.

Experienced injectors have recommended massage after injection, together with higher dilution, smaller amounts per session, placement under rather than in the skin, and at least six weeks between sessions, to reduce complications [2]. Massage is one part of that package, not a fix for poor placement; dilution and depth are decided in the clinic before the first injection.

Technique

How do you massage after Sculptra?

With clean hands, using firm circular movements over the treated area, about five minutes at a time. Use a little moisturiser or a gentle cleanser so fingers glide rather than drag the skin.

  • Cheeks and mid-face: two or three fingers in small circles over the cheekbone and below it, working outwards.
  • Temples: fingertips in small circles, with gentle pressure; temples are often tender for a day or two.
  • Jawline and chin: small circles along the line that was treated, supporting the jaw with the other hand.
  • Pressure: firm enough to move the tissue, never painful; ease off if it hurts and mention it at review.
  • What not to do: do not massage areas you were told to leave alone, and do not pinch or knead lumps hard.

If you are unsure where the product sits, ask for a photo or a marked diagram before you leave; the routine is shown and written down at each session.

First days

What should you avoid after Sculptra?

Strenuous exercise, saunas and alcohol for the period advised, usually the first day or two, and heavy pressure on the area outside the massage routine.

Make-up can usually go on several hours after treatment if the skin is intact [1]. Facials, microneedling and energy treatments on the same area wait until the clinic says so.

“The massage is not a ritual. Sculptra is a suspension of particles, and the first few days decide whether they settle evenly or gather in one place, so I ask people to do it properly and to tell me early if anything feels uneven.”
— Dr Sin Yong, Medical Director, IN Eternity Clinic
The fade

Why does Sculptra look full and then disappear?

The fullness on day one is mostly water and swelling, not the result. It fades within a week, and the face looks much as it did before. That is expected, not a sign of failure.

Poly-L-lactic acid works by prompting collagen, which takes weeks to months to build [3]. That is why sessions are spaced weeks apart and the next dose is decided only after the last one has shown its effect. Plans commonly use two or three sessions; the maker describes two to three sessions over a few months [1], and it has been studied in randomised trials [5]. How the course is costed is in Sculptra price in Singapore.

Lumps

Are lumps normal after Sculptra?

Small bumps you can feel but not see are fairly common in the first weeks and often soften with time and massage. Visible, firm or growing lumps are not normal and should be seen.

The maker notes that small bumps can sometimes be felt when pressing, and that larger lumps, with or without inflammation, have been reported [1]. Rarely, inflammatory lumps appear months or later [4]; these need assessment rather than more home massage. At IN Eternity Clinic a lump is examined and often scanned with ultrasound to see its size and depth before anything is done. The detail is in Sculptra side effects and nodules.

When to call

Which signs after Sculptra need a review?

Contact the clinic with a photo if you notice any of the following. Most are uncommon and easier to manage early.

  • Swelling that increases after the second day instead of easing.
  • A lump you can see, or one that is growing, red or tender.
  • Redness, warmth or pain that spreads.
  • A new lump weeks or months after treatment.
  • Any skin colour change or blanching over the treated area.
At assessment

What Dr Sin Yong looks for

  • Whether the face needs volume at all, and where, before any vial is planned.
  • Thin, mobile areas such as the lips and right around the eyes, which are avoided.
  • Earlier filler or collagen stimulator, mapped with ultrasound where useful.
  • How the face responded to the last session before more product is added.
  • Any lumps at review, and whether they are product, swelling or something else.
  • Your ability to do the massage routine, and any skin condition that makes it difficult.

Questions

Frequently asked questions

What happens if I do not massage after Sculptra?

Uneven spread of the particles becomes more likely, which raises the chance of small lumps. If you missed days, tell the clinic at review rather than massaging harder later.

Can I massage too much after Sculptra?

Follow the routine you were given. More pressure or longer sessions do not add benefit and can make tender areas sorer.

When can I wear make-up after Sculptra?

Usually several hours after treatment, if the skin is intact. Use clean brushes and gentle removal.

When can I exercise after Sculptra?

Avoid strenuous exercise for the period advised, usually the first day or two. Gentle walking is fine.

Why does my face look the same a week after Sculptra?

The early fullness was swelling and water. Collagen builds over weeks to months, so the change is judged at review, not in the first week.

Can I have a facial after Sculptra?

Wait until the clinic advises, usually a couple of weeks, and avoid deep massage or energy treatments on the area unless they were planned together.

Are small bumps after Sculptra normal?

Small bumps you can feel but not see are fairly common and often soften. Visible, firm, growing or tender lumps should be reviewed.
