Overfilled Face Correction

Undoing the heavy, puffy look without starting from zero

The overfilled face is rarely one bad injection. It is years of small top-ups that each made sense on the day.

In short · Doctor’s answer

An overfilled face ("pillow face") is the heavy, puffy, slightly expressionless look that builds up after repeated filler top-ups, usually in the cheeks, under the eyes, the lips and the chin. It is corrected in stages: an ultrasound map of what is where; hyaluronic acid dissolved zone by zone with small doses; collagen stimulators or inter-layered products that cannot be dissolved mapped and fragmented with the Time Freeze Laser LCLR® protocol in selected cases; then two to four weeks of settling before the face is reassessed. The last step is the important one: lifting the descended tissue with the same laser usually replaces most of the volume that the filler was standing in for.

What the research says
  • Hyaluronic acid fillers can persist far longer than their labelled duration, particularly in low-mobility zones, so repeated top-ups accumulate (clinical imaging series; Wortsman 2015).
  • Complications and over-correction relate to volume and plane rather than the product (DeLorenzi 2013).

Causes

How a face becomes overfilled

Filler is labelled to last six to eighteen months, but in the cheeks and under the eyes it often persists for years. Each top-up is added to product that has not gone. Add the tendency to treat every hollow with volume, including hollows that are really shadows cast by descent, and the face fills forward and sideways. The cheeks widen, the under-eyes puff, the lips lose their shape, the chin broadens, and expressions flatten because the product sits where muscles should move.

Assessment

What the assessment looks at

  • Which zones are overfilled, and by what material (records if available; ultrasound regardless).
  • How much of the heaviness is filler and how much is descent, fat or fluid.
  • Whether any zone has a true volume deficit that will remain once the excess is gone.
  • What the person wants back: their own face at rest, not a smaller version of the current one.
Correction

Correction in stages

The sequence
StepWhat happensTiming
1. MapUltrasound of each zoneVisit 1
2. Dissolve HASmall doses, zone by zone, under guidanceVisit 1, repeated as needed
3. Fragment non-HALaser on mapped stimulator deposits, selected casesVisit 1 or 2
4. SettleSwelling and deflation resolve2–4 weeks
5. LiftLaser lifting of descended tissue, zone by zoneFrom week 3
6. RefineSmall reversible volume only where a true deficit remainsMonth 2–3, if at all

Taking everything out in one visit is tempting and usually a mistake: the face looks deflated for weeks and the next decisions are made on a temporary picture. Staging keeps each step reversible.

Afterwards

What the face needs afterwards

Most overfilled faces were filled to cover descent. Once the product is gone, lifting the cheek, jowl and under-eye tissue with the LCLR protocol tends to restore the shape that the filler was imitating, and skin that was stretched by long-term volume is tightened in the same pass. Where a true deficit remains, a small amount of a reversible HA filler, placed deep, is enough. Costs depend on the zones, materials and sessions and are set out at the consultation; the consultation fee is S$120 and is waived when a treatment is started.

Questions

Frequently asked questions

Can an overfilled face be fixed?

Yes, in stages: ultrasound mapping, staged dissolving of hyaluronic acid, laser fragmentation of material that cannot be dissolved, a settling period, then lifting rather than re-filling.

How long does correction take?

Usually two to three months from the first visit to the refined result: dissolving and fragmentation in the first weeks, two to four weeks of settling, lifting from the third week onwards.

Will I look worse before I look better?

For a week or two after dissolving, the face can look deflated and tired. This passes as swelling settles and the tissue recovers.

Why not dissolve everything at once?

Because the face then looks collapsed for weeks and decisions about what to add back are made on a temporary picture. Staged dissolving keeps each step reversible.

Will I need filler again?

Often less than expected, and sometimes none, once descended tissue has been lifted. Where a true deficit remains, a small reversible volume placed deep is enough.

What if some of the filler was Sculptra or Ellansé?

Those cannot be dissolved. They are mapped by ultrasound and, in selected cases, fragmented with the laser protocol, then re-mapped before any further session.
