Thermage for the eyes uses a small Eye Tip to deliver monopolar radiofrequency heat to the eyelid and surrounding skin, tightening existing collagen and prompting new collagen over three to six months. It suits mild upper-lid hooding, crepey lids and fine lines around the eyes. It does not remove eye-bag fat, correct true ptosis or fill hollows.

The question

Can Thermage be used on the eyes?

Yes. Thermage, in its current FLX generation, has a dedicated Eye Tip for the eyelids and the skin around the eyes. It is one of a small number of radiofrequency treatments designed for use on the eyelid itself, rather than only along the brow and cheek. Searches for “eye Thermage” or “Thermage eyelids” usually come from people with mild hooding, crepey lids or fine lines who want to avoid eyelid surgery.

It is a good tool for a specific problem. Heavy-looking eyes can come from loose eyelid skin, a brow that has dropped onto the lid, a weak lid-lifting muscle, protruding fat, or hollowness under the eye. Thermage helps the first and, to a degree, the second. It does not help the others, and treating the wrong cause is the most common reason people are disappointed.

Age changes around the eye also tend to come together. In the thirties and forties, fine lines and slightly crepey lower lids are often the first sign. Later the outer brow descends, the upper-lid skin loosens and folds, and the fat pads beneath the lower lid become more prominent while the cheek below loses volume. Eye Thermage is most useful early in that sequence, when the skin itself is the main change and the underlying structure is still holding.

How it works

How does Thermage tighten eyelid skin?

Monopolar radiofrequency passes an electrical current from the treatment tip through the tissue to a return pad on the body. As the current meets the natural resistance of the skin, it produces heat in the dermis. The surface is cooled at the same time, so the heat is concentrated below it.

Heat has two effects on collagen. Some fibres contract straight away, which can give a small immediate firming. More importantly, the controlled thermal injury signals fibroblasts to make new collagen over the following three to six months. Studies of the tissue after monopolar radiofrequency have shown collagen contraction and new collagen formation, and a multicentre study of periorbital treatment reported brow elevation and smoother lines in most participants at six months.

The eyelid is a special case because its skin is so thin, with almost no fat beneath. The Eye Tip is smaller, so it can follow the curve of the lid, and energy settings are chosen for that thin skin. Rather than a few high-energy passes, the established approach uses more passes at lower energy, guided by feedback on comfort and the visible tightening response, which has been associated with better tolerance and more consistent results.

How is the eye protected during treatment?

When treatment is limited to the brow, the crow’s feet and the skin over the orbital bone, the eyes are simply kept closed. When the eyelid itself is treated, extra protection is used. Numbing eye drops are placed first, and then a smooth plastic shield is slipped gently under the lids to cover each eye. The shield protects the eye from heat while the lid skin above it is treated, and is removed at the end.

Contact lenses are taken out beforehand and not worn for the rest of the day. Eye make-up is removed, and the area is cleaned. People with recent eye surgery, an active eye infection, dry eye that needs treatment, or a pacemaker or other implanted electronic device need specific advice first, and treatment is deferred in pregnancy.

After treatment, the eyes may feel gritty for a few hours from the shields and drops. Persistent pain, blurred vision or a red, watering eye that does not settle should be reported the same day.

The experience

Does eye Thermage hurt, and what is recovery like?

Each pulse gives a short burst of deep heat, preceded and followed by cooling and accompanied by vibration from the tip. Around the eye most people describe it as hot and prickly but manageable. Numbing cream is applied to the skin for the brow and outer-eye area, and the eye drops make the shields comfortable. Energy is adjusted pulse by pulse according to how you feel.

The lids may look pink and slightly puffy for a few hours, and mild swelling can last a day or two, more so in the morning. Bruising is uncommon. Most people return to work the same day or the next. Rare effects include small blisters, temporary surface marks or, very rarely, a small dip from fat loss, which is one reason energy is kept conservative on thin, lean eyelids.

Some tightening may be visible within days, but the main change comes from new collagen and develops over three to six months. One session is usual, with a review after that. Many people repeat at around a year to maintain the effect.

Alternatives

How does eye Thermage compare with other options at IN Eternity Clinic?

Focused ultrasound can treat the brow and forehead to lift the outer brow, but lines are kept away from the eye itself, so it is less suited to the lid skin. Does Thermage work on hooded eyes?

It can help mild hooding caused by loose upper-lid skin. Multicentre studies of monopolar radiofrequency on the eyelids reported reduced hooding in many participants. It is less effective when the hooding comes from a dropped brow, heavy folds of excess skin or a weak lid-lifting muscle, and those causes are checked at assessment before treatment is recommended.

Is eye Thermage safe for the eyes?
