Ultherapy Prime is the newest generation of the Ultherapy system made by Merz Aesthetics. It was launched in the United States in September 2024 as the successor to the original platform, which had been in use for more than a decade. Searches for “Ultherapy Prime Singapore” have risen since, mostly from people trying to work out whether Prime is a different treatment, a stronger one, or simply a new machine.
The honest answer is that it is a new platform for the same treatment. Both generations use micro-focused ultrasound with visualisation, usually shortened to MFU-V. Both place tiny points of heat at fixed depths beneath the skin, including the SMAS layer, while leaving the surface intact. What Merz changed is the hardware and software around that principle: the imaging display, the speed of the workflow, the operating system and the feel of the handpiece in the doctor’s hand.
This page explains those differences without exaggerating them, and then turns to the question that matters more: whether focused ultrasound is the right tool for your face in the first place.

Focused ultrasound works like a magnifying glass for sound. The transducer converges ultrasound energy to a small focal point at a set depth, where it raises the temperature of a tiny volume of tissue to the point at which collagen contracts. Laboratory work on the technique showed that these thermal coagulation points can be placed precisely within the dermis and deeper layers while the skin surface above stays undamaged.
Each transducer is built for one depth. The 4.5 mm depth reaches the SMAS, the fibromuscular sheet that a surgical facelift lifts and repositions; 3.0 mm and 1.5 mm work in the deeper and more superficial dermis. A treatment is a map of lines laid along chosen vectors at chosen depths, so planning matters as much as energy.
After treatment two things happen. Some tightening comes from the immediate contraction of collagen, and more comes from remodelling, as fibroblasts lay down new collagen around the treated points over about two to three months. In a rater-blinded study of facial and neck treatment, most participants showed measurable brow lift at three months; reviews since then describe a moderate, gradual tightening effect rather than a surgical change.
The “V” in MFU-V is the visualisation. Before energy is released, the same transducer shows an ultrasound image of the layers beneath it, so the operator can confirm good contact and avoid placing lines over bone or where the tissue is too thin. Both the original Ultherapy and Ultherapy Prime have this feature. Prime improves the picture; it did not introduce it.

Three points are worth drawing out. First, the sharper image helps the operator see tissue layers and contact more clearly, which is useful in thin areas and around the jawline. It does not change how tissue responds to the heat once it is delivered. Second, a quicker workflow mainly shortens time in the chair. It is not the same thing as more lift. Third, the body clearances reflect new studies submitted by the manufacturer; they do not mean that Prime lifts a face more than the earlier system.
Merz has also said it will end support for the original platform in 2027, so over the next few years clinics that use Ultherapy are likely to move to Prime as a matter of course. If the generation matters to you, it is reasonable to ask which system your treatment will be on. Focused ultrasound suits mild to moderate laxity: an early jowl, a softening jawline, a brow that has started to sit lower, or loose skin under the chin and on the neck. It works well when there is a reasonable amount of soft tissue to tighten and the skin still has some recoil. It is used across a wide age range, but the typical candidate is someone noticing the first descent rather than someone with heavy folds.
It is less suitable for heavy, advanced sagging, which usually needs a surgical opinion; for fullness that is mainly fat or a short chin, which heat at the SMAS does not change; and for faces that are already lean or hollow. That last group matters. Heating fat-rich layers in a thin face can occasionally leave it looking more hollow, a recognised side effect of HIFU, Ultherapy and Thermage in lean faces, so it is discussed before any treatment.
This is where face shape guides the choice at IN Eternity Clinic. Each pulse is felt as a brief, deep prickling heat, strongest over bone such as the jawline and the brow. Numbing cream is applied beforehand, and settings and pacing are adjusted along the way. Comfort is broadly similar between the two generations, because the energy delivered to tissue is the same kind of energy.
Afterwards most people see mild redness that fades within hours. Some swelling, tenderness to touch, or small bruises can last a few days to a couple of weeks. Temporary numbness or a tingling patch along the jaw is uncommon and usually settles. Rarely, a superficial burn or welt appears where energy was placed too shallow, which is one reason the imaging and the operator’s judgement matter.
Change develops gradually over two to three months and is reviewed then, with photographs taken in the same light and position at the start. Most people have one treatment, and some return at around a year for maintenance. A review that shows little change is used to reconsider the plan, not simply to repeat it.
A newer platform is a reasonable thing to ask about, but it is a weaker guide than three other questions. Who assesses your face and does the treatment? How are the depths and the number of lines chosen for your anatomy? And what is the plan if focused ultrasound turns out not to be the right tool for you?
