How Long Does HIFU Last? Results, Ageing and Maintenance

It builds, then ageing carries on.

Medical Director Dr Sin Yong explains how long HIFU results last, what the studies report, what shortens or extends them and how maintenance is planned at IN Eternity Clinic.

In short

HIFU results build over two to three months as collagen remodels, and are often described as lasting around 12 to 18 months. Studies mostly follow people for up to a year, and improvement was still seen at that point. How long it holds depends on your starting laxity, age, sun and smoking, because ageing continues at its usual pace. See HIFU in Singapore.

Key facts
  • Up to a year: a systematic review of 16 studies found improvement in 92% of patients at day 90 on pooled investigator scores, continuing up to one year; longer-term data were not available [1].
  • Pooled effect: a meta-analysis of 17 studies with 477 participants found a moderate improvement, 2.74 on a 5-point scale, and most studies did not report long follow-up [2].
  • Peak and hold: in 28 women given one treatment, improvement peaked at three months and reduced facial and under-chin volume was still measurable at 12 months [3].
  • Who gains less: the review noted results declined with more skin laxity and a BMI above 30 [1].
  • Ageing continues: in 186 pairs of identical twins, the twin who smoked longer or had more sun exposure looked older [5].
Why it lasts

Why do HIFU results last months rather than days?

Because the change is made by your own tissue. HIFU focuses ultrasound into tiny points below the skin, where the temperature briefly passes about 60°C. Collagen at those points contracts, and the repair that follows lays down new collagen in the deep dermis and the SMAS support layer [4].

That is why the result is not seen on the day. It builds over the following two to three months, and is judged at review rather than in the mirror the next morning. At IN Eternity Clinic HIFU is done on the CLASSYS Ultraformer MPT, at depths of 1.5 and 3.0 mm for the dermis and 4.5 mm for the SMAS.

What studies show

How long does HIFU last according to the studies?

Most studies stop at about a year. A systematic review of 16 studies pooled investigator ratings for 337 people: at day 90, 92% showed improvement in tightening or wrinkles, and this continued up to one year. Patient ratings were milder but rose from 42% at 90 days to 53% at 360 days. The authors note that longer-term data are not available [1].

A meta-analysis of 17 studies found a moderate effect and the same gap: most studies did not report long follow-up [2]. A prospective study of 28 women followed for 12 months found that improvement peaked at three months, collagen signal on skin ultrasound peaked at nine months, and reduced volume under the chin was the most durable change [3]. On the clinic's HIFU page, results are often described as lasting around 12 to 18 months; individual results vary.

WhenWhat is usually seen
Day of treatmentMild redness or swelling; little visible change
Two to three monthsChange from collagen remodelling is easiest to see [1][3]
Nine to twelve monthsImprovement still reported in studies that followed this far [1][3]
Beyond a yearFew studies; ageing continues at its usual pace [1]
MaintenanceConsidered at around six to twelve months at IN Eternity Clinic, only if it would add something
What changes it

What makes HIFU last longer or wear off sooner?

The starting point matters most. The review above noted that results declined with greater skin laxity and with a BMI above 30 [1]. Very loose skin may gain little, and a lean or hollow face can lose volume it needs, so these faces are planned differently.

After treatment, the same things that age skin keep working. In a study of 186 pairs of identical twins, longer smoking and more sun exposure were each linked with an older appearance, and the effect of sun grew with age [5]. Daily sunscreen, not smoking and a steady weight help any result hold; repeating HIFU early does not speed the result and adds heat to tissue still remodelling.

At IN Eternity

How is HIFU maintenance planned at IN Eternity Clinic?

It is often a single fuller session, reviewed at two to three months with photographs in the same light. Maintenance is considered at around six to twelve months, and only if it would add something. Many people repeat about once a year; some less often. When sessions stop, the face simply continues to age at its usual pace.

Where HIFU is not the right tool, another approach is discussed, such as VF Lift on Volnewmer for a leaner face or Time Freeze Laser LCLR® for a fuller lower face. See HIFU in Singapore, HIFU results over time and how long aesthetic treatments last.

At assessment

What Dr Sin Yong looks for

  • Where the laxity sits: jawline, under the chin, brow or neck.
  • How much tissue lies between skin and bone, since a lean face can hollow.
  • Whether fat, bone structure or skin quality is driving the change instead.
  • Previous HIFU, threads or filler in the area, and when they were done.
  • Sun exposure, smoking and recent weight change, which affect how long change holds.

General clinical observations used at consultation; they describe what is checked, not any individual result.

Questions

Frequently asked questions

How long does HIFU last?

Results build over two to three months and are often described as lasting around 12 to 18 months, varying between people. Studies have mostly followed patients for up to a year, with improvement still seen at that point.

When will I see HIFU results?

A little tightening may be felt early, but most change appears over two to three months as new collagen forms. Results are judged at review, not on the day.

How often should HIFU be repeated?

At IN Eternity Clinic maintenance is considered at around six to twelve months, only if it would add something. Many people repeat about once a year and some less often.

What makes HIFU wear off faster?

Marked skin laxity, higher body weight, smoking, sun exposure and normal ageing all reduce how long a result appears to hold. HIFU does not stop ageing.

Does HIFU last longer if I have more sessions close together?

No. Repeating it before the tissue has remodelled does not speed the result and adds heat to tissue that is still repairing, so sessions are spaced.

Is HIFU permanent?

No. The collagen made is your own, but the face keeps ageing. Maintenance is planned individually rather than promised.
