Under-eye fullness is described as one thing, but it comes from five quite different sources. Bulging fat from behind the eye forms a true bag. A hollow below the eye casts a shadow that makes normal fullness look like a bag. Loose skin folds into crepe. Fluid comes and goes. Pigment, veins and thin skin make the area look dark and heavy. Many people have two or three of these together.
A few simple observations help. If the bulge grows when you look up and flattens when you look down, fat is likely. If tilting your face towards a ceiling light softens the darkness, a hollow is casting a shadow. If it is worst on waking and better by the afternoon, fluid plays a part. If gentle stretching leaves a brown colour, there is pigment. A doctor confirms this in person, from several angles and lights.

Where the main problem is a groove at the tear trough, replacing a small amount of lost volume reduces the shadow. At IN Eternity Clinic this is done with a soft hyaluronic acid filler that binds little water, placed deep on or near the bone, usually with a fine blunt cannula. Hyaluronic acid is used under the eye because it can be dissolved with hyaluronidase, a prescription enzyme, if needed. Firmer collagen-stimulating fillers are not placed in the tear trough.
The volumes are small, and restraint matters. Overfilling creates a puffy ridge, and filler placed too superficially can show as a bluish tint (the Tyndall effect). Delayed swelling after tear trough filler is well recognised, especially in people prone to fluid retention. In mild fat bulging with a deep groove below, a little filler in the groove can soften the step between lid and cheek; with a larger bulge it usually makes things look worse. When lower-lid skin has lost firmness, it folds into fine crepe and can sag slightly over the fat below. Radiofrequency and laser treatments heat the dermis, causing immediate collagen contraction and gradual new collagen over three to six months. Monopolar radiofrequency has been studied specifically for periorbital tightening.

Brown pigment is treated with low-energy pigment lasers over several sessions, with settings kept conservative because eyelid skin is thin and Asian skin can darken after aggressive treatment. Puffiness that changes through the day often relates to sleep, salt, alcohol, allergies, thyroid problems, kidney problems or medicines. Procedures are not the first step; identifying the cause is. Swelling low on the cheek that comes and goes, sometimes called malar oedema or festoons, is particularly prone to worsening after filler, so filler is avoided.
Bulging orbital fat is a structural change. Non-surgical treatment can improve the skin and the groove around it, but it does not remove the fat.
