How the Face Ages

Facial ageing occurs simultaneously at multiple levels: bone resorption reduces skeletal support; deep fat atrophies and descends; ligaments weaken; skin loses collagen and elastin; pigmentation accumulates. Effective anti-ageing treatment must address the appropriate level for each concern.

The Three Axes of Anti-Ageing

Dr Sin Yong frames anti-ageing treatment across three axes: lifting (addressing tissue descent with energy devices or threads), volume (restoring lost volume with fillers and biostimulators), and surface quality (improving skin with lasers and injectables). Effective protocols address all three axes as needed.

Energy-Based Lifting

The VF Lift uses volumetric focused energy for SMAS-level lifting. Time Freeze Laser LCLR combines collagen remodelling lifting with lipo-remodelling. Thermage uses monopolar RF for broad dermal tightening. These address the structural root cause of ageing — tissue descent.

At what age should I start anti-ageing treatments?+

Prevention is always easier than correction. Skinboosters and Profhilo from the late 20s maintain skin quality. Collagen-stimulating energy treatments become most relevant from the mid-30s. Lifting protocols address descent from the early 40s.

A face rarely ages in just one way. Bone and fat pads gradually lose volume and shift, ligaments loosen, the skin itself becomes thinner and less elastic, muscles leave expression lines, and years of sun leave pigment and uneven texture. Research on the facial skeleton shows that even the bony frame changes over time, which is why simply tightening skin can look flat if support has been lost underneath.

Most people have two or three of these at once. The order matters: support and volume are usually addressed before skin quality, and pigment is typed before any laser is chosen, because melasma and sun spots behave differently in Asian skin.

Energy-based lifting

Focused ultrasound and radiofrequency: what they can and cannot do

Focused ultrasound (HIFU, including Ultraformer MPT and Ultherapy) places small heat points at set depths, including the SMAS support layer beneath the skin. Monopolar radiofrequency such as Thermage and the CLASSYS Volnewmer heats the deeper skin more broadly. Both prompt gradual collagen remodelling, so change builds over two to three months rather than appearing on the day.

They suit mild to moderate laxity. Where skin is very loose, the change is modest, and a surgical opinion may be the more honest recommendation. Dr Sin Yong will say so at assessment. VF Lift – Vertical Facelift is his radiofrequency protocol on Volnewmer, planned along vertical vectors that follow how a particular face has descended.

Injectables

Anti-wrinkle injections, fillers and collagen stimulators, used with restraint

Anti-wrinkle injections soften lines caused by muscle movement and can be dosed lightly so expression stays natural. Hyaluronic acid fillers replace volume where it has been lost and can be dissolved if needed. Collagen stimulators work more gradually, and most cannot be dissolved, so the plan is conservative.

The most common problem seen in clinics is not too little filler but too much, placed to chase lines rather than restore support. If you already have filler that feels heavy or looks puffy, an assessment with ultrasound may show that filler correction is needed before anything is added.

Skin quality

Skin boosters and resurfacing for texture, hydration and fine lines

Skin boosters place hyaluronic acid or polynucleotides into the skin itself to support hydration and repair; examples include Rejuran, Plinest and Juvelook. RF microneedling and fractional lasers work on texture and fine lines by controlled micro-injury and remodelling. These treatments improve how skin looks and feels; they do not lift.

What tends to matter most in your 30s, 40s, 50s and beyond

30s: sun protection, pigment control, early expression lines and skin quality. Prevention does more than any single procedure. 40s: early volume loss and the first signs of laxity along the jawline. What is the most natural-looking non-surgical anti-ageing treatment?

There is no single answer, because natural results come from treating the cause that is actually present and doing a little rather than a lot. For many people that is light anti-wrinkle injections plus skin quality work; for others it is support with focused ultrasound or a collagen stimulator. An assessment decides which.

Can non-surgical treatments replace a facelift?

Not for marked laxity. Focused ultrasound and radiofrequency give a gradual, modest tightening that suits mild to moderate laxity. Where skin is very loose, a surgical opinion is more appropriate, and Dr Sin Yong will say so.

How long do non-surgical anti-ageing results last?

It depends on the treatment. Anti-wrinkle injections usually last a few months, hyaluronic acid fillers often around a year or more depending on the area and product, and collagen changes from energy-based lifting develop over months and are usually maintained with periodic sessions. Ageing continues, so plans include maintenance.

At what age should I start anti-ageing treatments?

Daily sun protection and good skincare matter from your 20s. Procedures make most sense once a specific change bothers you, not at a set age. Many people in their 30s need only skin quality and pigment care.
