Heat, cold or needles: three ways to reach fat.

Microwave heating, controlled cooling and radiofrequency microneedling all aim at stubborn local fat, but they differ in what else they do for skin. Dr Sin Yong explains the differences at IN Eternity Clinic on Orchard Road.

Onda Pro uses Coolwaves microwaves to heat fat while cooling the skin, and also acts on cellulite and laxity. Fat freezing (cryolipolysis) cools fat until fat cells die off gradually, with little effect on skin. The 4K Body Lift uses radiofrequency microneedling on Secret Duo to treat fat and tighten skin in mapped zones. None is weight loss.

Three mechanisms

How does each treatment reach fat?

Onda Pro (microwaves). Onda, from DEKA, uses microwave energy at 2.45 GHz delivered through a system called Coolwaves. Different tissues absorb electromagnetic energy differently, and at this frequency the energy is taken up preferentially in the fat layer while the skin surface is actively cooled. The heat damages fat cells, which the body then clears, and also heats the fibrous septae and deeper skin, which is why Onda is used for cellulite and laxity as well as fat.

Fat freezing (cryolipolysis). Fat cells are more sensitive to cold than skin, nerves or muscle. A cooling applicator, often with suction to draw a fold of tissue in, holds the area at a controlled low temperature for a set time. Fat cells in the treated zone undergo a gradual, programmed death over the following weeks and are cleared by the body. The skin itself is largely unaffected, for better and for worse.

4K Body Lift (radiofrequency microneedling). The 4K Body Lift is a protocol developed by Dr Sin Yong on the CLASSYS Secret Duo platform. Insulated microneedles deliver radiofrequency heat into mapped zones at the depth where the fibrous septae meet the superficial fat, then a second, shallower pass treats the dermis so the skin follows the new contour. It treats fat and skin quality in the same session.

Fat freezing

Where does fat freezing fit?

Cryolipolysis is well studied for reducing a pinchable bulge of fat, for example on the lower abdomen, flanks or under the chin, in people near a stable weight. Its strength is simplicity; its limit is that it acts on fat alone. Where skin is already loose, removing the fat beneath it can leave the skin looking slacker, and a separate tightening treatment may then be needed.

Common after-effects are redness, bruising, swelling, numbness and an ache that can last a few weeks. A rare complication, paradoxical adipose hyperplasia, causes the treated fat to enlarge and harden months later and usually needs surgical correction. Where does Onda fit?

Onda Pro suits people whose concern is a mix: a layer of stubborn fat with some dimpling or a softening of skin over it, on the abdomen, flanks, thighs, buttocks or arms. The applicator is kept moving over the area, so the sensation is a deep, building warmth rather than a single hot spot, and the skin is cooled continuously. Most people return to normal activities the same day, with redness, tenderness or a firm feel in the area for a few days.

A course of sessions is usually planned, spaced weeks apart, and the change develops as fat cells are cleared and the fibrous network remodels. IN Eternity Clinic is a designated DEKA Centre of Excellence, and Onda Pro is one of its DEKA systems.

4K Body Lift

Where does the 4K Body Lift fit?

The 4K Body Lift is chosen where definition and skin quality matter as much as fat: a lower abdomen with loose skin after pregnancy, inner thighs, the upper arms, the flanks or the area under the chin. Because the energy is delivered through needles at chosen depths, the treatment can be mapped zone by zone, with more energy where fat and laxity are greatest and less where the tissue is thin.

Numbing is applied beforehand. Afterwards the skin shows redness and pinpoint marks for a few days and may feel swollen or tender; most people return to daily activities the same day. Typically one to three sessions are planned about six weeks apart, with change developing over the following months.

How Dr Sin Yong decides

The starting question is what is actually there. Pinching and examining the area shows how thick the fat layer is, how loose the skin is, and whether cellulite or stretch marks are present. A firm, pinchable bulge with good skin above it can respond to several approaches. Fat with loose or crepey skin above it is better served by treatments that also heat the septae and dermis, such as Onda or the 4K Body Lift. Mostly loose skin with little fat needs a tightening plan rather than fat reduction.

Some people are better served by other routes. Significant weight to lose is a matter for diet, exercise and medical weight management first. Fat inside the abdomen, around the organs, is not touched by any surface treatment. Marked skin excess, for example after large weight loss or several pregnancies, needs a surgical opinion. Change from all three is gradual and varies between people, and it is reviewed with standardised photographs and measurements.

Safety

Who should not have these treatments?

Pregnancy, active skin infection in the area, metal implants or implanted electronic devices near the area for heat-based treatments, and some medical conditions are reasons to avoid or modify treatment. Cold-related conditions such as cryoglobulinaemia or cold urticaria rule out fat freezing. Your history, medicines and any previous procedures in the area are reviewed before a plan is made.

Realistic expectations are part of safety. These treatments change contour in a targeted area gradually and to a degree that varies between people. They suit people who keep a stable routine of activity and diet, and they are reviewed with standardised photographs and tape measurements rather than judged on the bathroom scale.

Is Onda better than fat freezing?

What is the 4K Body Lift?

Is there downtime?

Most people return to normal activities the same day after all three. Onda can leave redness and tenderness for a few days; the 4K Body Lift leaves redness and pinpoint marks for a few days; fat freezing can cause numbness and an ache lasting a few weeks. Strenuous exercise may be paused briefly.

How much does body contouring cost?

Cost depends on the treatment chosen, the number and size of areas, the number of sessions and whether fat reduction and skin tightening are both needed. After assessment, Dr Sin Yong gives a written quote that lists each part of the plan.

Will the fat come back?

Fat cells that have been destroyed and cleared do not regrow. However, the remaining fat cells can enlarge if weight increases, which can change the contour. A stable weight, regular activity and a sensible diet help maintain the result, and reviews check that the contour is holding.

Who is not suitable for body contouring?

People who are pregnant, those with active infection in the area, people hoping to lose significant weight, and those with marked skin excess who need a surgical opinion. Cold-related conditions rule out fat freezing, and implanted electronic devices may rule out heat-based treatment.
