Muscle Toning Devices in Singapore: HIFEM and Emsculpt Explained

Contractions you cannot make yourself, and what they can and cannot change.

Electromagnetic devices make a muscle contract hard without any exercise. Dr Sin Yong explains how they work, what the evidence supports, and how body plans are chosen at IN Eternity Clinic on Orchard Road.

HIFEM devices such as Emsculpt use a rapidly pulsed magnetic field to make a muscle contract thousands of times in one session, more intensely than you can by choice. Imaging studies report thicker muscle and, less consistently, thinner fat over it. It is not weight loss. The basics

What is HIFEM, and how is it different from a workout?

HIFEM stands for high-intensity focused electromagnetic energy. An applicator strapped over the abdomen, buttocks or limb produces a magnetic field that switches on and off very quickly. A changing magnetic field induces small electrical currents in the tissue beneath it, and those currents trigger the motor nerves that tell a muscle to contract. The skin is not heated and nothing breaks the surface.

The difference from exercise lies in the pattern. When you do a sit-up, your brain recruits muscle fibres in rotation and lets each one rest. The device bypasses that control. Pulses arrive so close together that the muscle cannot fully relax between them, producing what manufacturers call supramaximal contractions. A single 30-minute session can deliver thousands of these. Most people describe it as an intense, rhythmic tightening, like holding a hard plank for a long time without being able to stop.

The idea is not new. Electrical muscle stimulation has been used in rehabilitation for decades. What changed is the strength of the stimulus and the fact that a magnetic field can reach deep muscle through skin and fat without electrodes stuck to the skin.

Electrodes and pads, the kind sold for home use, pass current through the skin, so the sensation at the surface limits how hard the muscle can be driven, and the current spreads less deeply. A magnetic field passes through skin and fat without that surface sting, which is why a clinic device can produce much stronger contractions in a large muscle such as the rectus abdominis or gluteus maximus. The muscle still has to be within reach of the applicator: a thick layer of fat increases the distance and dilutes what arrives.

The evidence

What do the studies actually show?

The published work falls into three groups. Animal studies, mostly in pigs, have examined muscle under the microscope after treatment and describe larger muscle fibres and signs that new fibres may form. Imaging studies in people, using MRI or ultrasound, have measured the abdominal wall at the start and end of a course and report thicker rectus muscle and, in many participants, thinner fat over it. Smaller studies have looked at satisfaction and waist measurements.

This is a reasonable body of evidence for a measurable effect on muscle. It has limits that are worth knowing. Many trials are small, short and funded by the manufacturer. The fat findings are less consistent than the muscle findings, and the size of change varies widely from person to person. Long-term data are thinner: muscle gained without ongoing loading tends to return towards baseline, as it does after you stop training.

The practical reading is that HIFEM can make a muscle that is already near the surface look firmer and better defined. It does not remove a significant layer of fat, it does not tighten loose skin, and it does not change body weight.

What does adding radiofrequency change?

Emsculpt, the first HIFEM device, works on muscle alone. Emsculpt NEO places a radiofrequency heating element in the same applicator, so fat is warmed at the same time as the muscle contracts. Heat is intended to injure fat cells and to make the muscle more receptive to stimulation. The trade-off is a warmer, more intense session and a little more to consider for people with sensitive skin.

The table is a simplification. Real plans often combine approaches, because a soft abdomen is usually a mix of muscle tone, a layer of fat and some loosening of the skin.

Suitability

Who is it for, and who should avoid it?

The people who tend to be happiest with HIFEM are close to their usual weight, exercise at least some of the time, and want firmer, more defined muscle in a particular area. A thin layer of fat over the muscle lets the change show. Some women ask about it after pregnancy for a softer lower abdomen; whether it is appropriate depends on how far the abdominal muscles have separated and whether a hernia is present, which is a medical examination rather than a sales question.

A simple way to think about a soft abdomen or buttock is in three layers. If you can pinch a thick fold, fat is a large part of the shape. If the skin creases and hangs when you lean forward, skin laxity is involved. If the area is soft but the pinch is thin and the skin snaps back, muscle tone may be the main factor. Most people are a mixture, and the proportions decide which treatment, or which order of treatments, is likely to help.

It is not suitable for anyone with a pacemaker, defibrillator or other electronic implant, metal implants close to the treatment area, or during pregnancy. Some intrauterine devices, recent surgery in the area, an untreated hernia, and certain heart or bleeding conditions also need a doctor’s opinion first. It is not a substitute for losing weight where that is the main issue, and it does not address loose skin.

Expectations

What does a course feel like, and how long does it last?

A course is usually a small number of sessions spaced a few days apart, with each session lasting around half an hour. You lie on a couch while the applicator is strapped in place. Intensity starts lower and is increased as the muscle adapts. Muscle soreness similar to that after a hard workout is common for a day or two. There is no wound and you can return to normal activity straight after.

Changes in muscle develop over the weeks after a course rather than on the day. They are maintained by keeping the muscle working, through exercise, periodic maintenance sessions or both. Stopping everything allows gradual return towards where you started. Photographs in standard positions are taken at assessment so that progress is judged fairly at review.

Can it fix my tummy after pregnancy?

It may help muscle tone, but it does not close a significant separation of the abdominal muscles or repair a hernia, and it does not tighten loose skin. The midline and abdominal wall are examined first. Treatment is generally deferred until after breastfeeding and once the body has had time to recover.

How many sessions are needed?

Is it safe with a copper IUD or metal implants?

Metal near the treatment area is a reason for caution, and some manufacturers list intrauterine devices as a contraindication for abdominal treatment. Electronic implants such as pacemakers are an absolute contraindication. Your full medical history, including any implants, is reviewed before any session is booked.

How much does muscle toning treatment cost in Singapore?

Will I lose weight?

No. Muscle stimulation changes tone and definition in the treated area. Body weight is governed by diet, activity and health, and any weight change during a course is coincidental rather than an effect of the device. If weight is the main concern, that is a separate medical conversation.
