Plain microneedling makes thousands of tiny punctures and relies on the skin’s repair response to them. RF microneedling adds a second stimulus. Each needle is also an electrode, and once the needles are at the chosen depth, a short burst of radiofrequency current heats the tissue around the tips. The heat creates small columns of coagulation in the dermis. The skin repairs these by laying down new collagen and elastin over the following weeks and months, which is the basis for its use in acne scars, pores and texture.
Because the energy is released below the surface, the epidermis is injured far less than with an ablative laser that vaporises the surface. For Asian skin this matters: the risk of post-inflammatory hyperpigmentation rises with surface injury and heat, so a method that does most of its work underneath is often easier to plan around. The risk is lower, not absent.
Three settings decide what a pass does: depth, which chooses the layer; energy and pulse duration, which decide how much coagulation forms; and the number and spacing of passes. Different zones of the same face often need different combinations.
Needle design adds another variable. Fully insulated needles carry current down the shaft without releasing it until the uncoated tip, so the surface is largely spared and the heat sits where the tip stops. Non-insulated needles release energy along their length, heating the superficial dermis as well, which can help surface texture but adds more surface injury. Platforms differ in which needle types they offer, and operators choose between them according to the target.

In bipolar RF, current passes between neighbouring needles, so heat is confined to small, predictable zones around and between them. This suits precise work on scars and pores, and it avoids the need for a return pad. In monopolar RF, current flows from the needles to a pad elsewhere on the body, so heat spreads further around and below the needle tips. That broader heating is used for tightening and for thicker skin, but it is less contained and is not used in people with pacemakers or similar implants.
Having both modes on one platform gives an operator more choices; having a fractional laser alongside RF gives different choices. Neither is automatically an advantage for a particular person. What matters is whether the mode chosen fits the target layer, the skin thickness and the risk profile of your skin.

Potenza, made by Cynosure, is known for offering both monopolar and bipolar RF in one platform, at two frequencies. Monopolar mode spreads heat more broadly; bipolar mode keeps it tight around the needles. A range of tips with different needle counts and insulation lets the operator move between superficial work, such as fine texture, and deeper work, such as scars and laxity.
Secret Duo, a CLASSYS platform, uses RF microneedling with insulated needles at selectable depths and adds a 1540 nm non-ablative fractional laser handpiece on the same system. The laser creates fine columns of heating from the surface downwards without removing the skin. Combining the two lets the scar itself and the skin around it be treated in the same session, with RF working at depth and the laser refining the surface layers.

Both platforms have their own advantages and both are used well in many clinics. Published comparative trials between specific RF microneedling brands are scarce, so claims that one platform outperforms another usually rest on experience rather than head-to-head evidence.

Less than people expect. A rolling scar tethered by fibrous bands will not lift much with RF alone, on any platform, until the bands are released by subcision. An ice-pick scar is a narrow, deep shaft that usually needs a focused method such as TCA CROSS or point treatment with a CO2 laser. Boxcar scars with steep edges respond to a combination of resurfacing and remodelling. Matching the method to the scar type is the step that most affects the outcome.
Then come the settings. Too shallow and the energy misses the scar floor; too much energy in thin skin increases swelling and pigment risk; too few passes under-treat. These choices are made by the doctor for each zone and adjusted between sessions according to how the skin responded. Two clinics using the same device can deliver quite different treatments, which is why it is reasonable to ask how your plan will be mapped rather than only which machine will be used.
It suits people with depressed acne scars, enlarged pores, rough texture or mild laxity who want improvement with a shorter recovery than ablative resurfacing. Active acne is usually settled first. People taking oral isotretinoin, a prescription medicine, need individual advice on timing; see isotretinoin and procedures. A tendency to keloid scars, active infection or cold sores in the area, pregnancy, bleeding disorders and gold or metal allergy need discussion. Monopolar modes are avoided in people with pacemakers and similar electronic implants. Melasma-prone skin can be treated, but heat is planned carefully because it can provoke pigment.

Numbing cream is applied for 30 to 45 minutes beforehand. During treatment you feel pressure from the needles and brief heat with each pulse. Day 0: redness and a warm, tight feeling, sometimes with pinpoint bleeding or swelling. Days 1 to 3: redness fades, the skin may feel like sandpaper and faint grid marks may be visible. Week 1: most people look close to normal, with fine flaking. Weeks 2 to 4 and beyond: collagen remodelling continues quietly, and the next session is usually planned about four to six weeks later.
