It can, for the right double chin. HIFU (high-intensity focused ultrasound) tightens tissue. Under the chin it is most useful for fullness that comes mainly from loose skin and soft support layers, with a modest layer of fat. In that situation it can firm the area and sharpen the angle between the chin and neck, and studies also report a measurable, modest reduction in fat.
It is much less helpful when the fullness is mainly a large volume of fat, fat lying deeper beneath the neck muscle, or a chin that sits back. A quick self-check gives a rough idea. Look straight ahead in a mirror, then down. If the fullness mostly appears as a fold when you look down, and you can gather loose skin between finger and thumb, laxity is likely to be a large part of it. If there is a soft, thick pad even when your head is level, fat is involved. If the area looks full even though you are slim and your chin seems to sit behind your lower lip in profile, structure is likely to be the main driver. A doctor’s examination is still needed, because most necks show a mix.

Focused ultrasound converges energy to small points at a set depth below the surface. At each point the temperature rises enough to contract collagen and start a healing response, while the skin surface is left intact. Over the next two to three months, new collagen forms and the tissue firms.
Under the chin, different depths are used for different layers: shallower settings for the dermis, and deeper settings for the fibrous support tissue and the fat that lies just above the platysma, the thin sheet of muscle in the neck. Heat in fat can damage some fat cells, which the body then clears gradually. That is why HIFU, primarily a tightening treatment, can also reduce a modest amount of fat under the chin.
A study using triple-depth HIFU under the chin measured tightening and reduced submental fat with three-dimensional imaging, and a 2025 study in Asian patients reported a similar pattern with mild, temporary side effects. Earlier work on micro-focused ultrasound in the lower face showed tightening along the jaw and under the chin over several months. The effect is moderate and gradual, not surgical.

HIFU usually suits adults whose weight is stable, with early loosening under the chin and along the jaw, enough soft tissue to tighten, and realistic expectations. It suits less well if most of the volume is fat, if the chin is set back, or if the neck is very lean. In a lean neck and lower face, heating fat that you do not have to spare can make the area look hollow or older, a recognised side effect of focused ultrasound and other heat-based lifting in lean faces.
A neck with deep fullness beneath the muscle, a low hyoid bone, or marked excess skin is better assessed for a surgical opinion. Dr Sin Yong will say so at consultation rather than offer a treatment that cannot reach the cause.

When the chin itself sits back, adding projection with chin enhancement changes the angle under the chin directly and can reduce the look of fullness more than any tightening. Where a pinchable fat layer is the main issue, energy that reaches fat, such as microwave or radiofrequency contouring, is considered. Each line is felt as a series of short, deep, prickling heat points. The jawline and the area just under the chin bone are the most sensitive, because energy close to bone is felt more. Numbing cream is applied beforehand, and settings and pacing are adjusted along the way. A session for the area under the chin and along the jaw usually takes under an hour.
Afterwards there may be mild redness for a few hours, and swelling or tenderness under the jaw for a few days. Small bruises are uncommon. Occasionally a patch of numbness or tingling along the jaw, or slight temporary weakness of the lower lip from irritation of a nerve branch, can occur; these usually settle over weeks. Most people return to work the same day.
Change builds over two to three months as collagen remodels. One session is typical, sometimes followed by a second after review. Photographs taken in the same position at the start make the review fair.

They do different jobs. HIFU mainly tightens, with some fat reduction; fat-focused treatments act more on the fat layer. If your double chin is mostly loose skin, HIFU may suit; if it is mostly fat, a fat-focused approach makes more sense. Many people have both features, and the plan reflects that.
The tightening develops over two to three months and often lasts around a year, after which some people choose maintenance. Fat cells that have been cleared do not return, but remaining fat cells can enlarge with weight gain. Keeping your weight stable, protecting the skin from sun and not smoking help results last.
