In short

Lip embroidery can soften the look of dark lips, but it covers colour rather than removing it. Neutralising with a warm, orange-toned pigment can cancel some blue or purple, yet it adds ink and often heals muted. Where lips are dark from excess brown pigment, laser for lip pigmentation is usually the sounder first step, and lip blush is added once the lips have settled. See lip blush in Singapore.

Key facts
  • Many causes: a review groups dark lips into physiological colour, inherited syndromes, inflammation, hormonal disease, drugs and chemicals, and benign or malignant growths [1].
  • Smoking: in 30,118 Swedish adults, oral pigment was seen in 21.5% of smokers against 3.0% of non-users, and fell to non-user levels about three years after stopping [2].
  • Asian skin: in 467 Thai and Malaysian people, inherited pigment was the main finding, but smokers still had more pigmented surfaces than non-users [3].
  • Laser: in 30 people treated with Q-switched 532 nm laser for even, inherited lip darkness, 30% had more than 75% improvement and most needed one session [4].
  • Ink can darken: short-pulse lasers turned some pink, red, white and flesh-toned cosmetic tattoo inks black; iron oxide was the likely culprit [6].
Why lips are dark

Why are some lips naturally dark?

Lip colour is a mix of two things: blood showing through a thin surface, which gives pink and red, and melanin, which adds brown. How much melanin the lips make varies between people and ethnic backgrounds, so an even, slightly darker lip, often darker on the upper lip and along the border, is common in Singapore and is not a disease. A review of lip darkening groups the causes into physiological colour, inherited syndromes, inflammation, hormonal disease, drugs and chemicals, and benign or malignant growths [1].

Several causes are everyday ones. Smoking stimulates the pigment cells of the lips and gums; in a study of 30,118 adults, oral pigment was seen in 21.5% of smokers against 3.0% of people who did not use tobacco [2]. Sun reaches lips that sunscreen usually misses. Repeated irritation, from licking, biting or a reaction to lipstick, balm or toothpaste, leaves pigment behind as it settles. Some medicines deposit pigment in the lips and mouth.

Rule out first

Which dark patches on the lips should be checked before any tattoo?

Most dark lips are harmless, but a few patterns need a doctor before colour goes on. A single flat brown spot on the lower lip is usually a benign labial macule; one that is new in adulthood, growing, irregular or bleeding should be examined with a dermatoscope first, because tattoo ink over it hides the very changes that would need watching. Laser should not be used on it either until it has been diagnosed. See checking a spot before removal.

Several small dark spots on the lips and inside the mouth, especially with spots on the fingers, tiredness, low blood pressure or bowel symptoms, can point to an inherited syndrome, a hormonal condition such as adrenal insufficiency, or a medicine, and the review above describes how age of onset, extent and other symptoms guide that work-up [1]. These are uncommon. They are the reason the history comes before the colour chart.

Lip neutralisation

What is lip neutralisation, and what are its limits?

Lip neutralisation, also sold as dark lip neutralization or colour correction, places a warm pigment, often orange, peach or salmon, into lips that look bluish, purple or cool brown. It borrows colour theory: orange sits opposite blue, so a warm layer can cancel some of the coolness. Once that has healed, a pink or coral lip blush is often added on top. Done carefully on mildly cool lips, it can make the final colour look warmer and less grey.

Its limits are worth knowing before you start. It does not remove melanin; it adds a second pigment over it, so lips do not become lighter, only differently coloured, and the result often heals muted or nude rather than pink. Over strongly brown lips the layers can mix and heal brownish, grey or uneven. There are no published trials of lip neutralisation, so the number of rounds it takes and how it ages are not known from evidence. And many cosmetic inks contain iron oxide: in a study of laser-darkened cosmetic tattoos, pink, red, white and flesh-toned inks turned black on contact with short-pulse lasers, likely as iron oxide converted to a black form [6]. Ink placed today can make laser for the lips harder later.

When is laser for lip pigmentation the better first step?

When the darkness is brown pigment in the lip, and especially when you would like lighter lips rather than different-coloured ones. Q-switched and picosecond lasers break melanin into fragments the body clears. In 30 people with even, inherited lip darkness treated with Q-switched 532 nm laser and followed for at least six months, 30% had more than 75% improvement, another 37 to 43% had good improvement, and most needed only one session; cold sores reactivated in two people and three developed mottled lighter patches [4]. In 20 Thai patients given five sessions of a 755 nm picosecond laser, 70% reported moderate to marked improvement six months later [5]. These are small studies, and results vary.

Lip blush alone can be reasonable where lips are pale, uneven or only mildly dusky and the border has faded, with no pigment problem underneath. For smoker’s lips, stopping first is sensible where possible: in the Swedish study, pigment fell to the level of non-users about three years after quitting [2], and laser done while smoking continues tends to need repeating. Laser for the lips is planned by Dr Sin Yong; see lip pigmentation treatment and pico vs Q-switched laser.

At IN Eternity

How many sessions do dark lips need at IN Eternity Clinic?

The order here is pigment first, colour second. Dr Sin Yong examines the lips, with a dermatoscope for any single spot, and asks about smoking, lip products, medicines and cold sores. Benign brown pigment is treated with Q-switched 532 nm or 1064 nm, or picosecond 785 nm, on the DEKA TORO, or with the Fotona StarWalker, often over one to three sessions about four to six weeks apart; smoker’s pigment and deeper pigment can need more. Anyone prone to cold sores may be prescribed preventive antiviral medicine.

Lip blush is offered at IN Eternity Clinic. Your lips, cold-sore history and medicines are assessed at consultation, and the pigment work itself is done by the clinic’s embroidery specialist, not a doctor. Lip blush is planned once the lips have settled, usually a few weeks after the last laser session, as a first session and a touch-up at about six to eight weeks. On a more even base the shade can usually be softer and less pigment is needed. Cosmetic tattooing in Singapore is mostly offered as a beauty service; here a reaction or a colour that heals wrong is assessed by a doctor in the same clinic.

What Dr Sin Yong looks for

  • Whether the darkness is even inherited colour, smoker’s pigment, sun, irritation or a medicine.
  • Any single spot that is new, changing, irregular or bleeding, examined before anything else.
  • Spots inside the mouth or elsewhere that suggest a medical cause.
  • Cold-sore history, and any old lip tattoo or neutralising ink already in the lips.
  • Smoking and lip products, since pigment returns if the cause continues.

Questions

Frequently asked questions

Does lip embroidery work for dark lips?

It can soften how dark lips look, but it covers colour rather than removing it. Over strong brown pigment the tint often heals muted, grey or uneven. Where the darkness is pigment in the lip, laser usually comes first, and lip blush is added once the lips have settled, usually with a softer shade.

What is lip neutralisation?

Lip neutralisation places a warm orange, peach or salmon pigment into cool, bluish or purple lips to cancel some of the coolness before a pink or coral colour is added. It changes the hue but does not remove melanin, so lips do not become lighter, and there are no published trials of how well it lasts.

Is dark lip neutralization in Singapore safe?

It is cosmetic tattooing, mostly offered as a beauty service in Singapore, with the usual tattoo risks of infection, allergy and cold sores. A specific concern is that some cosmetic inks turn black when lasered later, which can make future lip laser harder. A medical check of the lips and history first is sensible.

Is lip blush good for dark lips?

Lip blush adds a soft tint and defines the border; it does not treat dark pigment. On lips that are only mildly dusky it can work well. On clearly brown lips, lightening the pigment with laser first lets the colour go onto a more even base and usually heal closer to the chosen shade.

Can smoker lips be lightened?

Often, with laser over a course of sessions, but pigment tends to return if smoking continues. In one large study, smoking-related oral pigment fell to the level of non-smokers about three years after stopping, so stopping first and treating what remains is a reasonable plan.

How many sessions are needed for dark lips?

Laser for lip pigment is often one to three sessions about four to six weeks apart, sometimes more for smokers or deeper pigment. Lip blush follows a few weeks later as a first session and a touch-up at about six to eight weeks. The number depends on the cause and is reviewed at each visit.

Can lip blush be lasered off if it heals too dark?

Often it can be lightened, but carefully. Pink, red and flesh-toned cosmetic inks can turn darker when hit by short-pulse lasers, so a small test area is treated and reviewed first, several sessions are usually needed, and complete clearance is not always possible.
