Scalp Micropigmentation Aftercare: Healing Day by Day
Medical Director Dr Sin Yong explains scalp micropigmentation aftercare: healing day by day, washing, sweating, sun and shaving, and which signs need a review at IN Eternity Clinic.
WhatsApp to Enquire →Key facts ↓Scalp micropigmentation aftercare is mostly about protecting fresh dots while they crust and heal over about a week: keep the scalp dry for the first few days, then wash gently without scrubbing; avoid heavy sweating for several days and swimming or saunas for about two weeks; do not pick crusts; hold off shaving until they have gone; and keep out of strong sun. See scalp micropigmentation in Singapore.
- Inflammation settles: in a tattooed human skin model, the strong inflammatory reaction after tattooing subsided about 4 days after treatment, followed by a healing phase [1].
- Advice varies: of 700 tattoo aftercare instructions, 14.9% advised topical antibiotics, few said to wash hands before touching, and only 19.4% said when to see a doctor [2].
- Infection: skin infections after tattooing are most often caused by Staphylococcus aureus or Streptococcus pyogenes; acute local inflammation is the commonest complication [3].
- Timing of reactions: a 2024 review notes that tattoo reactions can appear immediately or emerge several years later [5].
- Sessions: in one series of 22 patients, three weekly sessions were followed by a touch-up one month after the last [6].
What does SMP healing look like day by day?
Each session heals in the same pattern: redness first, then fine crusts, then flaking and softening. The timings below are from the clinic's SMP page and are typical rather than fixed. Laboratory work matches the early phase: in a tattooed human skin model, the inflammatory reaction subsided about four days after tattooing and a healing phase followed [1].
| When | What you may see | What to do |
|---|---|---|
| Day 0 to 1 | Redness and slightly raised dots; the scalp may feel tender | Keep it dry; do not touch or scratch |
| Days 2 to 5 | Tiny crusts over the dots, which look darker and sharper than they will heal | Leave crusts alone; gentle washing once advised |
| Days 5 to 10 | Crusts flake off; dots soften and lighten | Do not pick; hold off shaving until crusts have gone |
| Weeks 2 to 3 | Settled enough to judge; may look lighter or patchy | Review and next session, usually one to three weeks after the last |
| After the final session | Colour and density settle over about a month | Sun protection long term; touch-ups every few years |
A layer that looks lighter or uneven at two to three weeks is expected. That is why density is built over sessions, and why the next layer is planned only once the last one can be judged.
When can you wash your hair after scalp micropigmentation?
Keep the scalp dry for the first few days after each session; the clinic tells you the timing on the day. After that, wash gently: lukewarm water, a mild shampoo worked with the palms rather than the nails, no scrubbing, then rinse and pat dry with a clean towel. Avoid exfoliating acids, scrubs and strong anti-dandruff products until the crusts have gone, because they lift fresh pigment with the surface skin. Wash your hands before touching the area.
Advice from different providers often conflicts. In an analysis of 700 tattoo aftercare instructions, recommendations varied widely: 14.9% advised topical antibiotics, few told people to wash their hands, and only 19.4% said when to see a doctor [2]. At IN Eternity Clinic written aftercare is given after each session; do not add creams or antibiotic ointments unless the clinic advises them. More general guidance is on aftercare at IN Eternity Clinic.
Can you sweat, go to the gym or swim after SMP?
Not straight away. In the first days, sweat and friction can disturb the healing dots and pull pigment out, so heavy exercise and the gym are avoided for several days, and swimming, saunas and steam rooms for about two weeks. In Singapore's heat, that includes long outdoor walks at midday and hot, physical work; light activity in air-conditioning is usually fine. A loose cap can be worn once advised, but avoid tight hats or helmets that rub the treated area.
Sun matters during healing and long after. Keep the scalp out of strong sun while it heals, then use a hat or scalp sunscreen. A systematic review found that sunlight breaks down tattoo pigments, which is the main reason SMP fades [4]. Once healed, normal sweating does not wash pigment out.
When can you shave after scalp micropigmentation?
Hold off shaving the treated area until all the crusts have gone, usually about a week; then shave gently. Use a clean blade or an electric shaver with light pressure, go with the grain at first, and rinse rather than rub. Shaving over crusts pulls them off early and takes pigment with them, which shows as small pale gaps at the next review.
If you wear a buzz-cut look, the real hair around the pigment needs clipping every few days so the two match. Between sessions, keep the hair at the length agreed at your design appointment, so the next layer can be placed accurately. Women and others keeping longer hair do not shave; the hair is usually sectioned during sessions instead, and the same washing and sun advice applies.
What is normal after SMP, and what needs a review?
Mild redness, tenderness, tiny crusts and dots that look darker before they settle are all normal. Signs of infection or a reaction are not. Skin infections after tattooing are most often bacterial, usually Staphylococcus aureus or Streptococcus pyogenes, while acute local inflammation is the commonest complication overall [3]. Allergic and granulomatous reactions can appear soon after or emerge years later [5].
| Usually normal | Message the clinic for a review |
|---|---|
| Redness and tenderness on day 0 to 1 | Redness that spreads or worsens after day 2 |
| Fine crusts that flake by about day 10 | Pus, yellow crusting, increasing pain or warmth |
| Dots that look darker, then lighter | Swelling, fever or feeling unwell |
| Mild itch as crusts lift | Persistent itch, raised bumps or swelling of the dots, even months later |
| A lighter or patchy look at two to three weeks | Dots that look blurred or blue at any stage |
How many SMP sessions are needed, and how is aftercare followed up?
At IN Eternity Clinic a typical plan is two to three sessions, sometimes four for large areas or scars, spaced one to three weeks apart, then touch-ups every few years. The same aftercare applies after each session. Published protocols vary, for example three weekly sessions followed by a touch-up one month later in one series [6], so the spacing is set by how your scalp heals rather than by a fixed calendar.
Before the first session, Dr Sin Yong assesses the cause of hair loss and the scalp, and checks for conditions that slow healing or raise the risk of infection, such as poorly controlled diabetes or immune-suppressing medicines. Scalp micropigmentation is offered at IN Eternity Clinic. The cause of your hair loss, your scalp and the design are assessed at consultation, and the pigment work itself is done by our embroidery specialist, not a doctor. If healing does not go as expected, a doctor reviews it in the same clinic. See hairline embroidery, how long SMP lasts and what decides SMP cost. Suitability is decided at an in-person assessment.
What Dr Sin Yong looks for
- How the previous layer healed, including any patchy or lifted areas.
- Signs of infection, allergy or granuloma that need medical treatment.
- Scalp conditions such as seborrhoeic dermatitis, psoriasis or folliculitis that slow healing.
- Health factors such as diabetes or immune-suppressing medicines.
- Sun exposure and work or exercise habits that affect aftercare.
General clinical observations used at consultation; they describe what is checked, not any individual result.
Frequently asked questions
What does SMP healing look like day by day?
Day 0 to 1: redness and slightly raised dots. Days 2 to 5: tiny crusts, with dots looking darker. Days 5 to 10: crusts flake and dots soften. Weeks 2 to 3: settled enough to judge and ready for the next session. Colour settles about a month after the final session.
When can I wash my hair after scalp micropigmentation?
Keep the scalp dry for the first few days after each session, as advised by the clinic on the day. Then wash gently with lukewarm water and a mild shampoo, without scrubbing, and pat dry. Avoid exfoliating or strong anti-dandruff products until the crusts have gone.
Can I sweat after SMP?
Avoid heavy sweating, the gym and hot physical work for several days, and swimming, saunas and steam for about two weeks, because sweat and friction can disturb the healing dots. Once healed, normal sweating does not wash the pigment out.
When can I shave my head after scalp micropigmentation?
Wait until all the crusts have fallen off, usually about a week, then shave gently with light pressure. Shaving over crusts pulls them off early and can leave pale gaps in the pigment.
Can I wear a cap after SMP?
A loose cap can be worn once the clinic advises it, which also helps with sun protection. Avoid tight hats, helmets or headbands that rub the treated area while it heals.
How many SMP sessions are needed?
Usually two to three, sometimes four for large areas or scars, spaced one to three weeks apart so each layer heals before the next. Touch-ups follow every few years as the pigment fades.
What are the signs of infection after scalp micropigmentation?
Redness that spreads or worsens after the second day, pus or yellow crusting, increasing pain, warmth, swelling or fever. Message the clinic promptly; a reaction is assessed by a doctor at IN Eternity Clinic.
How do I protect SMP from the sun?
Keep the scalp out of strong sun while it heals, then use a hat or a scalp sunscreen long term. Sunlight is the main cause of fading, and the scalp gets a lot of sun when hair is kept short.
