Cellulite Creams vs In-Clinic Devices: What Each Can and Cannot Do
Medical Director Dr Sin Yong explains what caffeine and retinol creams can do for cellulite, what the trials show, and how devices are used at IN Eternity Clinic.
WhatsApp to Enquire →Key facts ↓Creams with caffeine or retinol can make cellulite look slightly smoother while you keep using them, but they act near the surface and do not reach the fibrous bands that pull the skin down. In-clinic devices deliver energy beneath the skin. At IN Eternity Clinic, cellulite is graded first, then treated with Onda Pro microwave energy, adding the 4K Body Lift for loose skin. See cellulite treatment in Singapore.
- How common: cellulite affects an estimated 85 to 98 in 100 women after puberty, so it is a normal finding rather than a disease [1].
- Caffeine cream: in 15 people using a 3.5% caffeine cream twice daily for six weeks, a visual cellulite score improved by about one fifth and thigh circumference fell by 0.7 cm; there was no comparison group [2].
- Retinol cream: in a left-right, placebo-controlled trial in 15 women, six months of retinol made the skin more elastic, but the lumpy-bumpy look showed little or no response [3].
- Overall evidence: a systematic review of 67 studies found only 19 were randomised and placebo-controlled, and no clear evidence of good efficacy for any cellulite treatment, devices included [4].
- Device evidence: the Onda Coolwaves advisory board set out settings and patient selection, and noted that long-term efficacy and safety have not yet been fully evaluated [5].
What causes cellulite, and why does that matter for creams?
Cellulite sits in the layer under the skin. Vertical fibrous bands anchor the skin to deeper tissue; where fat bulges up between them and the skin is thin or loose, the surface dimples. A review of its physiology describes four main explanations: differences in skin structure between women and men, the fibrous bands themselves, changes in small blood vessels, and inflammation [1].
A cream is applied to the surface. Some ingredients reach the upper skin, but the bands sit lower, in the fat layer. That is why a cream can change how skin looks and feels without releasing the tethering. Exercise and weight loss change fat volume, not the bands, which is why lean people can still have cellulite.
What can caffeine and retinol creams actually do?
Caffeine. Caffeine is added to slimming creams because it is thought to encourage fat breakdown. In a small study, 15 people applied a 3.5% caffeine cream to the thighs and upper arms twice a day for six weeks. A visual cellulite score improved by about one fifth, thigh circumference fell by 0.7 cm, and itching and flushing were common but mild [2]. There was no comparison group, so part of the change may come from massage, moisturising or measurement, and the authors called for larger trials.
Retinol. In a left-right trial, 15 women applied retinol to one thigh and a placebo cream to the other for six months. The retinol side became about 11% more elastic, and the effect was clearer where cellulite showed only as a fine "mattress" pattern; the lumpy-bumpy look showed little or no response [3]. Retinol can also irritate the skin.
| Option | Works on | What to expect | At IN Eternity Clinic |
|---|---|---|---|
| Caffeine cream | Surface and upper skin | Slight smoothing in an uncontrolled study; lasts while used [2] | Advice only |
| Retinol cream | Skin elasticity | Firmer skin; little change in deeper dimples [3] | Advice only |
| Massage, dry brushing | Fluid in the skin | Smoother for hours to a day or two | Advice only |
| Onda Pro (microwave energy) | Fibrous bands, localised fat, skin firmness | Gradual change over a course; maintenance needed | Main tool |
| 4K Body Lift (RF microneedling) | Loose skin and the zone where bands meet fat | Added where laxity is part of the dimpling | Added where needed |
How do in-clinic devices differ from creams?
Devices deliver energy beneath the surface, where the bands and fat sit. Onda Pro is a DEKA device that sends microwave energy at 2.45 GHz into the fat layer while the skin surface is cooled, aiming to reduce localised fat and remodel the tissue that tethers the skin. The 4K Body Lift is Dr Sin Yong’s protocol on the Secret Duo: insulated radiofrequency microneedles treat mapped zones where the fibrous bands meet the fat, then a shallower pass treats the skin so it can follow the new contour.
Devices are not a cure either. A systematic review found that most cellulite studies, including those of lasers and radiofrequency, had important flaws, and found no clear evidence of good efficacy for any treatment [4]. The Onda advisory board notes that long-term efficacy and safety are still to be fully evaluated [5]. In practice a course of sessions spaced weeks apart, each about 30 to 60 minutes, gives gradual change that is judged on photographs about three months after the last session, and cellulite returns without upkeep.
Is a cream enough, or is a device worth considering?
It depends on the grade and what drives it. Dimpling seen only when the skin is pinched may need nothing more than a stable weight, exercise and, if you like, a cream for comfort. Dimpling visible when standing is unlikely to change with a cream. Where loose skin is part of the picture, a device plan has to address that too.
| Grade | What it looks like | Usual direction |
|---|---|---|
| Mild | Smooth when standing; dimples when pinched | Weight stability and exercise; a cream if wanted; Onda Pro if it bothers you |
| Moderate | Dimpling or orange-peel look when standing | Onda Pro course, sometimes extra passes for fat |
| Severe | Deep dimples, raised areas and folds when standing | Onda Pro with the 4K Body Lift for laxity; realistic goals agreed |
Onda Pro is not used in pregnancy or breastfeeding, over metal implants or an implanted electronic device in the area, or on infected or broken skin, and treatment is better started once weight is stable. See Onda Pro and the 4K Body Lift.
What Dr Sin Yong looks for
- How the dimpling looks standing, lying and pinched, which separates bands from fat and laxity.
- Fat thickness in each area, which changes how Onda Pro is used.
- Skin laxity, which may mean adding the 4K Body Lift.
- Creams, massage or devices already tried, and what changed.
- Weight trends, pregnancy plans, recent surgery, implants and devices in the area.

Frequently asked questions
Do cellulite creams work?
They can make the skin look and feel a little smoother while you use them, but they act near the surface and do not release the fibrous bands that cause dimples. Trials of creams are small, and the effect fades when you stop.
Does caffeine cream get rid of cellulite?
No. In a small study without a comparison group, a 3.5% caffeine cream used twice daily for six weeks improved a visual score by about one fifth and reduced thigh circumference slightly. It does not change the structure behind cellulite.
Is retinol helpful for cellulite?
In a placebo-controlled trial in 15 women, six months of retinol made the skin more elastic, but the lumpy-bumpy look showed little or no response. It may suit mild cellulite and can irritate the skin.
Can a device remove cellulite permanently?
No. Devices aim for a smoother look over a course, with maintenance, because the bands, fat and skin behind cellulite are normal anatomy. Results vary between people and are judged on photographs.
Can I keep using a cream during device treatment?
Creams, massage and dry brushing are harmless as comfort measures and can usually continue, unless the skin is irritated. Any change to your routine around sessions is explained at your visit.
What does IN Eternity Clinic use for cellulite?
Dimpling is graded first. Onda Pro microwave energy is the main tool, with the 4K Body Lift on the Secret Duo added where loose skin is part of the dimpling, over a course of sessions.
